Shock Risk Screen Calculator

Check whether entered shock-review factor rows are recorded, missing, or unknown without classifying risk or producing a work conclusion.

Formulas

  • \(\text{Recorded factors}=\text{Count of rows marked recorded}\)
  • \(\text{Missing factors}=\text{Count of rows marked missing}\)
  • \(\text{Unknown factors}=\text{Count of rows marked unknown}\)
  • \(\text{Required missing factors}=\text{Count of required rows marked missing}\)
  • \(\text{Factor completeness}=\text{Recorded factors}/\text{Total factors}\times100\%\)
  • \(\text{Review note is complete only when missing and unknown counts are zero}\)

The Shock Risk Completeness Calculator produces a factor completeness percentage for a reviewed shock-risk method. It identifies whether the factor records needed for that review have been entered, marked missing, or left unknown.

The result is used at the documentation stage of an electrical safety review, before a shock-risk classification, energized-work decision, boundary determination, or qualification conclusion is made. A complete record does not establish that a circuit is safe to approach or service. It confirms only that the factors entered for review have a recorded status.

The calculator uses the following result fields:

  • Total factors
  • Recorded factors
  • Missing factors
  • Unknown factors
  • Required missing factors
  • Factor completeness
  • Review note

A review with a 100% completeness result and no required missing factors is shown as All entered factors recorded.

Review Factors

Each row under Review factors represents one factor used by the reviewed method. The row contains:

FieldElectrical documentation use
FactorIdentifies the condition, item, or decision factor being reviewed
Factor statusRecords whether the factor is available, missing, or unknown
Required factorIdentifies whether the reviewed method requires the factor to be present
Source referenceRecords the supporting source, such as a drawing, equipment document, field verification record, or approved assessment reference

The check is intended to prevent a risk conclusion from being based on incomplete inputs. For example, a shock-risk review may depend on equipment identification, nominal system voltage, source documentation, task conditions, or other factors defined by the method being used. If one of those required inputs is marked Missing, or if any row remains Unknown, the review record is incomplete even when other rows have been documented.

The calculator does not determine conductor ampacity, feeder loading, available fault current, incident energy, voltage drop, raceway fill, or equipment suitability. Those are separate electrical calculations or engineering determinations.

Factor Completeness Formula

The calculator counts all rows by Factor status and calculates the share marked Recorded.

\(\displaystyle \text{Factor completeness} = \frac{\text{Recorded factors}}{\text{Total factors}} \times 100\%\)

Where:

  • Total factors is the total number of factor rows entered.
  • Recorded factors is the number of rows with Factor status set to Recorded.
  • Missing factors is the number of rows marked missing.
  • Unknown factors is the number of rows marked unknown.
  • Required missing factors is the number of rows marked Missing where Required factor is Yes.
  • Factor completeness is reported as a percentage.

The Review note reflects record completeness. A complete entry requires zero Unknown rows and zero required rows marked Missing.

Calculation Example

A review contains one factor row:

FieldEntered value
FactorRequired factor
Factor statusRecorded
Required factorYes
Source referenceProvided

The calculator returns:

ResultValue
Total factors1 rows
Recorded factors1 rows
Missing factors0 rows
Unknown factors0 rows
Required missing factors0 rows
Factor completeness100%
Review noteAll entered factors recorded

The arithmetic is:

\(\displaystyle \frac{1\text{ recorded factor}}{1\text{ total factor}} \times 100\% = 100\%\)

This result confirms that every entered factor row has a recorded status and that no required factor is marked Missing.

Electrical Review Boundary

The Shock Risk Screen Calculator is a completeness check only. It does not classify shock risk or determine a work method, approach boundary, energized-work authorization, PPE selection, or worker qualification.

Those decisions require the applicable electrical safety procedure, verified equipment and system information, task-specific conditions, and the responsible employer’s or site’s approved process. A source reference should identify the actual basis for a recorded factor rather than merely indicating that a row was completed.

Field conditions can also change after the review record is prepared. Equipment labeling, disconnect position, backfeed sources, generator connections, photovoltaic systems, UPS outputs, control power, shared neutrals, and undocumented modifications may require the review factors and supporting references to be updated before electrical work proceeds.

FAQs

Does completeness classify shock risk?

No. It only reports whether the entered factor rows are recorded, missing, or unknown.

Why count required missing factors separately?

It makes missing inputs visible without converting the worksheet into a risk or work-practice decision.
